PISGS Disqualifying Offenses (Ohio Administrative Code 4501:7-1-19)

Criminal Conviction Determination Form (DPS 0179)

Ohio Revised Code 9.78(C) provides: "A licensing authority shall make available to the public on the licensing authority's internet web site a list of all criminal offenses of which conviction of that offense shall disqualify an individual from obtaining a license issued or conferred by the licensing authority"

A potential applicant seeking a determination of disqualification pursuant to O.R.C. 9.78 may submit a form requesting a determination. A determination request that does not include all the required details of the individual's criminal conviction(s) is incomplete. ODPS will not make a determination based on an incomplete request.
